Ensuring that the voices and lived experiences of patients and caregivers will lead to improved patient and health system outcomes? If so, please consider putting your/their name forward for one or both of the following opportunities to be involved as a patient or caregiver in local health care planning with the Central East Local Health Integration Network (Central East LHIN).
The Central East LHIN is one of 14 Ontario LHINs established by the provincial government. The Central East LHIN’s mandate is to plan, fund and integrate health care services in a defined geographic region which is comprised of Scarborough, the Region of Durham, Northumberland County, Peterborough City and County, Haliburton County and the City of Kawartha Lakes.
Through accountability agreements with health care service providers – Hospitals, Community Health Centres, Community Support Services, Community Mental Health and Addictions agencies and Long-Term Care Homes – the LHIN is ensuring that integrated, accessible and high quality health care services are available so that people can live healthier at home.
Two NEW opportunities are available to patients and caregivers who are interested in making a difference in the local health care system:
CENTRAL EAST LHIN PATIENT AND FAMILY ADVISORY COMMITTEE (PFAC) – deadline to apply October 24, 2017
Expanding patient engagement across the health care system is a key commitment articulated in the Patients First Act. A key element of this approach has been the establishment of Patient and Family Advisory Committees (PFACs) in each of Ontario’s 14 Local Health Integration Networks (LHINs).
The Central East LHIN’s PFAC aims to assist, at a system level, in shaping the LHIN’s programs, services and initiatives designed to improve care across the Central East LHIN region. The PFAC advises and collaborates with the Central East LHIN, its leaders, HSPs and staff regarding system-level policies, practices and strategy in the planning and delivery of patient and family-centred care across the Central East LHIN.
In late October 2016 the Central East LHIN actively recruited community members to participate in the Central East LHIN Patient and Family Advisory Committee (PFAC). Six individuals, who have been patients, or caregivers of patients in the Central East LHIN were invited to join the committee as inaugural members.
At present, the PFAC includes geographic representation from the Durham West and North East, Scarborough South and Scarborough North and the City of Kawartha Lakes.
With this Expression of Interest, we encourage patients and their family/caregivers who live within the Central East LHIN’s Northumberland County, Peterborough City and County and Haliburton County sub-regions and/or people who self-identify as a member of the francophone, indigenous, new immigrant and/or LGBTQ communities to complete and submit an Expression of Interest for consideration as a member of the system-level Central East LHIN Patient and Family Advisory Committee.

CENTRAL EAST LHIN SUB-REGION PLANNING TABLES – deadline to apply September 12, 2017
The Central East Local Health Integration Network (Central East LHIN) is seeking health system stakeholders – patients, caregivers, Indigenous representatives, Francophone representatives, New Immigrant representatives, Primary Care (Physicians, Nurse Practitioners), Specialist Physicians, Hospital sector representatives, Public Health Unit representatives, Municipal Services representatives, Home and Community Care services director representatives, Community Support Services sector representatives, Long-Term Care Homes sector representatives and Mental Health and Addictions sector representatives – to participate in local Sub-region Planning Tables.
The mandate of the seven Sub-region Planning Tables is to provide local leadership and joint accountability for innovative, integrated system redesign to collectively address health and service gaps, advance quality, and improve patient experience and outcomes within a local sub-region geography.
The seven sub-region geographies include (1) Scarborough North, (2) Scarborough South, (3) Durham West, (4) Durham North East, (5) Northumberland County, (6) Peterborough City and County; and (7) Haliburton County and the City of Kawartha Lakes.
Sub-region Planning Tables will work collaboratively within their sub-regions, and with one another through a Sub-region Steering Committee, to manage local priorities and improve equity of access to health care and supports for patients and caregivers.
